I have read and agree to abide by the following Open Mic rules: *

Time limit is 10 min for solo performers or 15 min for groups of 2 or more
Performers must check in next to the stage when they are "on-deck" or next in line on the sign up sheet.
You must have your instrument tuned and be waiting on the couch ready to get on stage and begin sound check as soon as your name is called by the host.
No food or drinks are allowed on the stage.
Possession or use of illegal drugs and excessive or underage alcohol use is absolutely prohibited at open mic.
Everyone is expected to be kind and respectful to each other at The Phoenix Clubhouse. Any kind of physical or verbal attack, threat or abusive remark/insult is absolutely prohibited. Violating this rule in any way will result in you being immediately trespassed from the premises.
This is an all ages event, please give a quick disclaimer before you begin if your material may be considered offensive, uncomfortable, or inappropriate for some viewers (language, trigger warnings, etc.)

Picture & Video Authorization: *

I authorize The Phoenix Clubhouse to record, livestream and take pictures during any public performance(s).
I authorize The Phoenix Clubhouse to use any pictures and/or videos captured by venue staff at their sole discretion. This includes posting the photos and videos publicly on our website, YouTube channel and social media sites or using them for other promotional purposes.
I understand that this authorization does not guarantee that any or all of my performance(s) will be recorded, or that any pictures or videos will be posted online or made available to me after my performance.

Each time you sign you sign up for open mic you will be asked whether you want pictures and videos taken during your performance. You may choose not to have any pictures or videos taken that week or to have only specific tracks recorded.
